All Seasons Mental Health has an immediate opening for a Community Based Rehabilitation Specialist (CBRS)/Case Manager.

A CBRS/Case Manager assists in the empowerment, growth, and development of individuals who are recovering from persistent mental illness. Through education and support, you will work to guide individuals and celebrate their successes as they strive to achieve their personalized goals in areas such as: basic living skills, medication management, finances, socialization in the community, and coping with their mental and/or physical disabilities. You'll work one-on-one with individuals to promote recovery; to integrate them with their community, and provide services aimed at helping the individual improve their quality of life. Community integration often includes linking them to various support resources and advocating on their behalf.

As a full-time member of the All Seasons Team you'll be provided benefits such as: paid time off, health insurance, 401k plan with matching, work-issued cell phone and laptop, and incentive pay.

We utilize both in-person and tele-health communications with our patients/clients which allows for greater flexibility for all participants (clients and team members). Pay Range: $17 - $23 per billable hour / DOE
