POSITION GENERAL SUMMARY:
Provides case management services to clients and their families; performs assessments, screenings, and referrals to community-based programs to address client and families’ needs; provides counseling and help navigating community resources; and, performs related work as required. This position supports The Bridge, Ada County’s Youth and Family Resource Center, Ada County Juvenile Services.
Essential Functions
+ Provides excellent customer service to engage youth and their families in program participation;
+ Performs screening assessments on referrals from law enforcement, schools and the community;
+ Refers clients for additional in-depth assessments when warranted;
+ Completes evidence-based risk/needs assessments;
+ Identifies and recommends appropriate services and programs available in the community to address client and family need;
+ Makes referrals for medical, psychological, legal, financial, shelter and employment services;
+ Assists individuals and families with crisis intervention;
+ Performs case management services on assigned cases and/or refers to a community resource specialist;
+ Monitors and ensures client’s progress with case plan;
+ Communicates and coordinates with other community programs;
+ Keeps supervisor informed of workload, and notifies supervisor of potential problems;
+ Collects, analyzes, and documents statistical data related to program assignments and utilization of community resources; and,
+ Assists in monitoring program budgets and providing information for future budget development.
ADDITIONAL FUNCTIONS:
+ Works with community-based resources; and
+ Performs other duties as assigned.
Job Requirements
+ Bachelor’s Degree from an accredited university in Social Work, Criminal Justice, Psychology or a related human services field required;
+ Experience working with vulnerable populations and/or social services preferred;
+ Knowledge of the principles and practices of social services;
+ Knowledge of individual and family crisis intervention techniques;
+ Knowledge of program development and needs assessment;
+ Knowledge of adolescent development issues;
+ Knowledge of social services;
+ Skill in creating a comfortable atmosphere for conducting client and family interviews and for facilitating group sessions;
+ Skill in conducting successful interviews;
+ Skill in making appropriate assessments;
+ Skill in developing and/or implementing case plans;
+ Skill in communicating with diverse populations;
+ Ability to use professional judgment in the application of appropriate policies and guidelines;
+ Ability to effectively work as a team member;
+ Ability to collaborate with a variety of agencies within the community;
+ Ability to plan and organize work;
+ Ability to understand and interpret laws and regulations;
+ Ability to communicate verbally and in writing;
+ Ability to effectively work with and counsel juveniles and /or adults;
+ Ability to establish and maintain rapport with individuals in crisis, including small children;
+ Ability to maintain confidentiality.
OTHER REQUIREMENTS:
+ Incumbent must have a strong desire to work with at-risk juveniles and their families;
+ Must adhere to the Code of Ethics, Values and Policies of Juvenile Services;
+ This position has been designated safety sensitive and therefore the incumbent is subject to random drug testing.
Work Environment & Physical Demands
+ The incumbent will be required to work flexible schedules, which may include work on some weekends, evenings, nights, and/or holidays;
+ Requires sufficient personal mobility and physical reflexes, to permit the employee to function in a general office environment and to physically respond to emergency situations in the field and office.
